When you choose Cooperative Home Care Associates (CHCA), you can be certain that you are receiving a worker that has been trained in illnesses/diseases such as diabetes, dementia, multiple sclerosis and so much more. In addition, our workers receive hands-on instructions and practicums in our enhanced classrooms. All workers receive quarterly continuing education training classes to maintain their skills and knowledge to date.

CHCA understands and empathizes with patients feeling lonely and/or limited with their mobility. Companion care is a non-medical service that focuses on improving the quality of life by providing companionship to individuals who are socially isolated. Companion care can include:
CHCA was selected by the Department of Health as a lead Fiscal Intermediary (FI) in NY City.
What is an FI? A Fiscal Intermediary is an organization that provides financial and administrative services on behalf of the consumer.
CDPAP puts you in charge of your home care! You get to choose, train, and guide your own caregivers! Your friend or family member gets paid to take care of you! And you get the care you deserve your way!
You work together with your health plan to determine the services that you need. You get to select your own Personal Assistant (family member or friend) and train them to care for you. In addition, this program allows you to create the schedule that’s best for you, your needs, and your lifestyle!

Peer MentorshipWe train our Home Care Workers to become Peer Mentors, who work with new and incumbent Home Care Workers. This one-to-one mentorship helps workers acquire the full range of skills needed to deliver high-quality care. |

Senior Aide ProgramOur Senior Aide Program provides interdisciplinary care teams with an in-depth view of an individual's case so they can identify underlying health issues and strengthen communication and reporting. |

Care CoordinatorsOur Care Coordinators work closely with nurses to address any observations or concerns reported by Home Care Workers or their families. This empowers the entire care team to provide each person with excellent care. |
